Families are facing housing instability, health barriers, and economic hardship without enough coordinated support.

The HART Act connects housing, health, workforce, and community resources into one practical support pathway.

Programs —

Our programs provide navigation, education, workforce readiness, family stabilization, and community resilience services. Our organization operates through a coordination and community engagement model that emphasizes collaboration rather than duplication of services through platforms such as Findhelp.org, and public health institutions throughout Maryland, Virginia and Washington D.C, Trained and certified as Community Health Supervisors, Community Health Workers, Critical Medical Needs Navigators and Licensed Food Managers in the State of Maryland. Engaging in Peer Support Specialist  Training and Mental Health First Aiders Youth, Teen and Adult Blended Curriculum Training and Mental Health First Aid Instructors. 



We work by:

Connecting residents with existing programs and opportunities

Coordinating referrals and support pathways

Providing participant-centered engagement

Supporting documentation and intake processes

Collaborating with community organizations and stakeholders

Building long-term stabilization infrastructure

Using lived experience to help inform community-centered solutions

Through Community Uplift and HART ACT initiatives, we continue developing systems that support housing stabilization, workforce participation, transportation coordination, education, and community engagement.

We help residents prevent crisis, access resources, build stability, and move toward long-term self-sufficiency. Impact is established when residents are reassured, not dependent on programs we help them navigate by educating residents about program resources not intake vulnerabilities.
